Admission

CNA Enrollment Requirements

  • Accuplacer test results (reading, arithmetic and WritePlacer)
  • Arizona Department of Public Safety fingerprint fingerprint clearance card: Level 1
  • Proof of health insurance
  • Physical exam report
  • Tuberculosis (TB) skin test results
  • Immunization record
  • Proof of American Heart Association BLS certification
  • Negative drug screen proof (Program staff will provide paperwork to you for drug screening.)
